Where can I find the surveys?

Where can I find the Pre-/Post- Surveys and Mental Health Referral Forms?

The Surveys and Mental Health Referral Forms are  bundled in the Measurement Toolkit. You can access that Toolkit by following these steps:
  1. When you first login to the Leader Portal, select "I am a Squad Advisor" 
  2. Next select the box near the top of the page that says "Measure Impact" (Please note that you can also access this same Measure Impact section by using the dropdown arrow next to "Advisors" at the top of the page
  3. Scroll about half way down the page to the section called "Measurement Toolkit". The right-most box is titled Survey Links. Press "View Links" and you'll be able to access all of the surveys and the Mental Health Referral Form.
