Where can I find the Pre-/Post- Surveys and Mental Health Referral Forms?
The Surveys and Mental Health Referral Forms are now all bundled together in the Program Evaluation Toolkit. You can access that Toolkit in one of 3 places:
- When you first login to the Advisor Portal, you can scrool down to the bottom of the page where one of the navigation boxes says "Program Evaluation Toolkit". You'll be able to find all of the surveys and the mental health referral forms there.
- Navigate to Getting Started then scroll down to "Step 5: 1st Member Meeting: Planning & Pre-Survey. The surveys can be found highlighted in the section that says "Program Evaluation Toolkit".
- The third location is in Squad Success. Navigate to Squad Success then scroll down to the section titled "Program Evaluation". Select the red bar that says, "Program Evaluation". Selecting that button will also take you to the Program Evaluation Toolkit where you can access Pre-/Post- Surveys and Mental Health Referral Forms
