What is the Mental Health Referral Form and when should I fill it out?

The Mental Health Referral Form is used to help your school and organization keep track of mental health related situations. The data captured in the Mental Health Referral Forms and the Pre and Post Surveys can be very valuable in helping your school see the effectiveness of Hope Squad's implementation in your school.

Any school counselor or Hope Squad Advisor that meets with any student who reports suicide-related concerns or any other mental health issues should submit a Mental Health Referral Form. The data from Mental Health Referral Forms and Hope Squad Member Surveys is available to authorized persons in your school upon request.
